Cinacalcet has been extensively studied in patients with secondary hyperparathyroidism associated with chronic kidney disease. Clinical studies have consistently demonstrated reductions in PTH and serum calcium levels and improved control of mineral metabolism. Studies in patients with parathyroid carcinoma and severe primary hyperparathyroidism have also demonstrated the ability of cinacalcet to reduce elevated serum calcium. Treatment requires regular monitoring of serum calcium and PTH because excessive calcium reduction may result in hypocalcemia.
Cinacalcet is used to treat secondary hyperparathyroidism in adults with chronic kidney disease who are receiving dialysis. It is also used to reduce hypercalcemia in adults with parathyroid carcinoma and in adults with primary hyperparathyroidism when parathyroid surgery cannot be performed or is contraindicated.
The active ingredient increases the sensitivity of calcium-sensing receptors in the parathyroid glands to calcium. This reduces PTH secretion and helps control calcium levels and other parameters of mineral metabolism.
The medication should be taken orally with or shortly after food. Tablets should be swallowed whole and should not be chewed or crushed. The dose is individualized by a healthcare professional according to the indication and laboratory results. Serum calcium and other relevant laboratory parameters should be monitored regularly during treatment.
